I am not sure that i understand the state Fon Shen is in. Looks like she misses some part s of her face? Leprosis? Unlikely, the hands should have been damaged before that. I don’t get it, but she is probably crazed at least.
I believe it is late stage syphilis, which would likely also be affecting her mind. She likely caught it from one of her night clients after she gave birth.
